By participating in the Game Changers Foundation Flag Football Tournament, I understand and agree to the following: 1. Assumption of Risk: I acknowledge that participating in the flag football tournament involves inherent risks, including but not limited to the risk of injury or mishaps. I understand the nature of the activities and voluntarily choose to participate, assuming all risks associated with such participation. 2. Release of Liability: I hereby release and discharge Game Changers Foundation, its directors, officers, employees, volunteers, and agents from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, or mishap that may occur during the flag football tournament. This release includes, but is not limited to, any personal injury, property damage, or wrongful death caused by the negligence, gross negligence, or intentional misconduct of Game Changers Foundation. 3. Indemnification: I agree to indemnify, defend, and hold harmless Game Changers Foundation, its directors, officers, employees, volunteers, and agents from any and all claims, demands, actions, or causes of action brought against them by any third party arising out of or related to my participation in the flag football tournament. 4. Medical Treatment: In the event of an injury or medical emergency, I authorize Game Changers Foundation and its representatives to obtain medical treatment for me or my child, including but not limited to, transportation to a medical facility, administration of first aid, and/or the engagement of emergency medical services. 5. Media Release: I grant Game Changers Foundation the irrevocable right and permission to use photographs, videos, or other media of me or my child for promotional purposes, including but not limited to, website content, social media, and marketing materials. 6 - Governing Law: This waiver shall be governed by and construed in accordance with the laws of [State/Country].
